Hospice Care
At Colorado Palliative & Hospice Care our mission is to serve others. Patients near the end of their life, family members trying their best to support their loved ones, caregivers and professionals striving to provide the best care possible need assistance and we want to provide that assistance!
Palliative Care
The PEACE program is a free community service dedicated to supporting individuals who may require hospice care in the future. It provides ongoing follow-up for those who are either ineligible for hospice or not yet ready, focusing on their overall well-being and connecting them with essential services
Medical Professionals
There are major differences in hospice relationships – let us help build one that benefits you and your patients. Our team has the programs and the procedures to make our relationship one that will be valued by you and your staff.
Testimonials
“The care, compassion, knowledge and support that my grandmothers team has provided her has been absolutely phenomenal! Being an out of state relative it eases my heart to know that she is in great hands and truly being looked after and cared for. They have gone the extra mile and extended help and support to our family as well, I couldn’t ask for a better team of people! I’m so thankful!”
Unlike many hospices owned by large national for-profit corporations, we are locally owned and operated, making decisions right here in our community. Our team lives and works in the areas we serve, and it shows in our reviews. Families consistently praise our compassionate care, personalized support, and deep commitment to the communities we call home.

Medicare
While Medicare sets the minimum required services for hospice care, Colorado Palliative & Hospice Care goes beyond the standard to provide exceptional, personalized support. We honor each patient’s unique life experiences and spiritual beliefs, ensuring they receive the highest level of care with dignity and respect.
Like nonprofit hospices, we accept Medicare payments at the same rate, while also helping reduce Medicare costs by providing care in the comfort of our patients’ homes.

Hospice Benefit
Among the many benefits available to our patients, hospice care is often the most misunderstood. Despite its incredible support and minimal cost, participation remains low. We believe this is due to a fundamental misunderstanding of what hospice truly offers.
